YOUR VISIT

Please Bring Along To Your Appointment:
1. The completed paperwork which may be printed and downloaded below.
2. Two forms of ID, one of which needs to have a photo (drivers license, state issued ID, passport, work badge, and credit card with photo
     suffice). Your insurance card is fine as your secondary ID.
3. Insurance cards
4. Your co-pay (if applicable).
5. List of all current medications and allergies.
6. X-ray films if done at other hospitals or facilities (you will also be responsible for returning them.)
7. Any medical records or test results pertinent to your treatment.
